Turn pthread_spinlock_t into a self-contained object. On uni-processor
configurations, interrupts are disabled in the lock/trylock operations
and the previous interrupt status is restored in the corresponding
unlock operations. On SMP configurations, a ticket lock is a acquired
and released in addition.
The self-contained pthread_spinlock_t object is defined by Newlib in
<sys/_pthreadtypes.h>.

This implementation is simple and efficient. However, this test case of
the Linux Test Project would fail due to call of printf() and sleep()
during spin lock ownership:

There is only limited support for profiling on SMP configurations.

We always build a C++ compiler and building with C++ does not effect
RTEMS or the runtime. This patch always enabled the support. There is
no need to manually enable it any more.
You can disable C++ with '--disable-cxx'.
If an architecture does not have a C++ compiler support is automatically
disabled.

This was obsolete and broken based upon recent time keeping changes.
Thie build option was previously enabled by adding
USE_TICKS_FOR_STATISTICS=1 to the configure command line.
This propagated into the code as preprocessor conditionals
using the __RTEMS_USE_TICKS_FOR_STATISTICS__ conditional.
