The use of CONFIGURE_APPLICATION_NEEDS_TIMER_DRIVER does not define
anything, so remove the <rtems/btimer.h> include.

The rtems_test_pause() and rtems_test_pause_and_screen_number() macros
had different implementations depending on the RTEMS_TEST_NO_PAUSE
define. This define was defined to 1 by default. The user was able to
change this via the undocumented --disable-test-no-pause configure
command line option.
Pausing tests and waiting for user input contradicts the goal of having
automated test runs. Remove this feature.

- Remove the printf support leaving the direct printk support configured
with TESTS_USE_PRINTK and all other output goes via a buffered vsniprintf
call to printk.
- Control the test's single init for functions and global data with
TEST_INIT and not CONFIGURE_INIT. They are now separate.

Notepads where a feature of RTEMS' tasks that simply functioned in
the same way as POSIX keys or threaded local storage (TLS). They were
introduced well before per task variables, which are also deprecated,
and were barely used in favor of their POSIX alternatives.
In addition to their scarce usage, Notepads took up unnecessary memory.
For each task:
- 16 32-bit integers were allocated.
- A total of 64 bytes per task per thread.
This is especially critical in low memory and safety-critical applications.
They are also defined as uint32_t, and therefore are not guaranteed to
hold a pointer.
Lastly, they are not portable solutions for SMP and uniprocessor systems,
like POSIX keys and TLS.

smartly as possible.
+ remove occurrences of two blank comment lines
next to each other after Id string line removed.
+ remove entire comment blocks which only exited to
contain CVS Ids
+ If the processing left a blank line at the top of
a file, it was removed.

successfully with maxixum number of priorities as 16 instead of 256.
This was done by temporarily modifying the score priority.h maximum.
This allowed testing of all API code to ensure that it worked
properly with a reduced number of priorities. Most modifications were
to switch from hard-coded maximum to using the API provided methods
to determine maximum number of priority levels.

* tmoverhd/dumrtems.h: Refactored and renamed initialization routines
to rtems_initialize_data_structures, rtems_initialize_before_drivers,
rtems_initialize_device_drivers, and
rtems_initialize_start_multitasking. This opened the sequence up so
that bootcard() could provide a more robust and flexible framework
which is easier to explain and understand. This also lays the
groundwork for sharing the division of available memory between the
RTEMS workspace and heap and the C library initialization across all
BSPs.
