This patch is a part of the BSP source reorganization.

Script does what is expected and tries to do it as
smartly as possible.
+ remove occurrences of two blank comment lines
next to each other after Id string line removed.
+ remove entire comment blocks which only exited to
contain CVS Ids
+ If the processing left a blank line at the top of
a file, it was removed.

* libchip/Makefile.am, libchip/shmdr/init.c, libchip/shmdr/mpisr.c,
libchip/shmdr/poll.c, libchip/shmdr/shm_driver.h: Update shared
memory driver to not use the clock ioctl to install a method to poll
for input. It now uses a Class API Timer which means we can eliminate
this special IOCTL from all clock drivers.
* libchip/shmdr/setckvec.c: Removed.

Renamed file shmsupp/intr.c in some BSPs to shmsupp/cause_intr.c to
avoid conflict with rtems/src/intr.c (Classic API Interrupt Manager).
