Simplify object name to identifier directives. Using
_RTEMS_Name_to_id() to implement the directives enables a tail call
optimization.
Change license to BSD-2-Clause according to file history.

The CONFIGURE_DISABLE_SMP_CONFIGURATION configuration option and
rtems_configuration_is_smp_enabled() were added during the SMP support
development cycle as a workaround to fix some testsuite failures in SMP
configurations. All use cases were replaced with tests for specific
conditions. The configuration option and test macro were undocumented.

Determine the default for CONFIGURE_INIT_TASK_INITIAL_MODES depeding on
whether RTEMS_SMP is defined or not.
In the tests, use CONFIGURE_INIT_TASK_INITIAL_MODES to explicitly
request RTEMS_NO_PREEMPT mode if necessary.

Statically allocate the objects information together with the initial
set of objects either via <rtems/confdefs.h>. Provide default object
informations with zero objects via librtemscpu.a. This greatly
simplifies the workspace size estimate. RTEMS applications which do not
use the unlimited objects option are easier to debug since all objects
reside now in statically allocated objects of the right types.

- Remove the printf support leaving the direct printk support configured
with TESTS_USE_PRINTK and all other output goes via a buffered vsniprintf
call to printk.
- Control the test's single init for functions and global data with
TEST_INIT and not CONFIGURE_INIT. They are now separate.

Enable the SMP configuration by default in case SMP is enabled. Add
configuration option CONFIGURE_DISABLE_SMP_CONFIGURATION to disable it
explicitly.
Add CONFIGURE_DISABLE_SMP_CONFIGURATION to all test which would fail
otherwise.

Move implementation specific parts of thread.h and thread.inl into new
header file threadimpl.h. The thread.h contains now only the
application visible API.
Remove superfluous header file includes from various files.

In these places use an alternative. Add and use
