- Remove the printf support leaving the direct printk support configured
with TESTS_USE_PRINTK and all other output goes via a buffered vsniprintf
call to printk.
- Control the test's single init for functions and global data with
TEST_INIT and not CONFIGURE_INIT. They are now separate.

This change adds rtems_printf and related functions and wraps the
RTEMS print plugin support into a user API. All references to the
plugin are removed and replaced with the rtems_printer interface.
Printk and related functions are made to return a valid number of
characters formatted and output.
The function attribute to check printf functions has been added
to rtems_printf and printk. No changes to remove warrnings are part
of this patch set.
The testsuite has been moved over to the rtems_printer. The testsuite
has a mix of rtems_printer access and direct print control via the
tmacros.h header file. The support for begink/endk has been removed
as it served no purpose and only confused the code base. The testsuite
has not been refactored to use rtems_printf. This is future work.

Call the test extension explicitly and wait for report output using a
barrier. This avoids problems with an early shutdown of the system.

Rename rtems_smp_get_current_processor() in
rtems_get_current_processor(). Make rtems_get_current_processor() a
function in uni-processor configurations to enable ABI compatibility
with SMP configurations.

Rename _SMP_Request_other_cores_to_perform_first_context_switch() into
_SMP_Request_start_multitasking() since this requests now a multitasking
start on all configured and available processors. The name corresponds
_Thread_Start_multitasking() and
_SMP_Start_multitasking_on_secondary_processor() actions issued in
response to this request. Move in source file to right place.
Rename PER_CPU_STATE_READY_TO_BEGIN_MULTITASKING into
PER_CPU_STATE_READY_TO_START_MULTITASKING.
Rename PER_CPU_STATE_BEGIN_MULTITASKING into
PER_CPU_STATE_REQUEST_START_MULTITASKING.
Rename _SMP_Request_other_cores_to_shutdown() into
_SMP_Request_shutdown().
Add a per-CPU state lock to protect all changes. This was necessary to
offer a controlled shutdown of the system (atomic read/writes alone are
not sufficient for this kind of synchronization).
Add documentation for Per_CPU_State.
Delete debug output.
New tests smptests/smpfatal01 and smptests/smpfatal02.
