- Remove the printf support leaving the direct printk support configured
with TESTS_USE_PRINTK and all other output goes via a buffered vsniprintf
call to printk.
- Control the test's single init for functions and global data with
TEST_INIT and not CONFIGURE_INIT. They are now separate.

This change adds rtems_printf and related functions and wraps the
RTEMS print plugin support into a user API. All references to the
plugin are removed and replaced with the rtems_printer interface.
Printk and related functions are made to return a valid number of
characters formatted and output.
The function attribute to check printf functions has been added
to rtems_printf and printk. No changes to remove warrnings are part
of this patch set.
The testsuite has been moved over to the rtems_printer. The testsuite
has a mix of rtems_printer access and direct print control via the
tmacros.h header file. The support for begink/endk has been removed
as it served no purpose and only confused the code base. The testsuite
has not been refactored to use rtems_printf. This is future work.

This partially reverts commit 1215fd4d9426a59d568560e9a485628560363133.
In order to support profiling of SMP locks and provide a future
compatible SMP locks API it is necessary to add an SMP lock destroy
function. Since the commit above adds an SMP lock to each chain control
we would have to add a rtems_chain_destroy() function as well. This
complicates the chain usage dramatically. Thus revert the patch above.
A global SMP lock for all chains is used to implement the protected
chain operations.
Advantages:
* The SAPI chain API is now identical on SMP and non-SMP
configurations.
* The size of the chain control is reduced and is then equal to the
Score chains.
* The protected chain operations work correctly on SMP.
Disadvantage:
* Applications using many different chains and the protected operations
may notice lock contention.
The chain control size drop is a huge benefit (SAPI chain controls are
66% larger than the Score chain controls). The only disadvantage is not
really a problem since these applications can use specific interrupt
locks and unprotected chain operations to avoid this issue.

rtems_chain_extract() with rtems_chain_explicit_extract() and
rtems_chain_insert() with rtems_chain_explicit_insert() on SMP

Add and use rtems_blkdev_request_done(). Block device transfer requests
must signal the completion status now with rtems_blkdev_request_done().
The return value of the block device IO control will be ignored for
transfer requests.
The first parameter of rtems_blkdev_request_cb is now the transfer
request structure.
Renamed rtems_blkdev_request::req_done to rtems_blkdev_request::done to
break third party drivers at compile time, otherwise this API change
would result in runtime errors.

The functions
o rtems_bdbuf_get(),
o rtems_bdbuf_read(),
o rtems_bdbuf_syncdev(), and
o rtems_bdbuf_purge_dev(),
use now the disk device instead of the device identifier. This makes
bdbuf independent of rtems_disk_obtain() and rtems_disk_release(). It
is the responsiblity of the file system to obtain the disk device. This
also reduces the overhead to get a buffer.
The key for the AVL tree uses now the disk device instead of the device
identifier. The pointer is interpreted as an unsigned integer. This
reduces the memory overhead and makes the comparison operation a bit
faster.
Removed function rtems_bdbuf_purge_major(). This function was too
destructive and could have unpredictable side effects.
