The TLS section symbols had wrong values in case of an empty TLS data

Instead of SPRG0 (= special purpose register 272) use the new global
symbol _PPC_INTERRUPT_DISABLE_MASK to store the interrupt disable mask.
The benefit is that it is now possible to disable interrupts without
further run-time initialization in boot_card().
At least on Freescale e500 cores this leads also to a faster execution
since the mfmsr and mfspr instruction require four cycles to complete.
The instructions to load the mask value can execute while the mfmsr is
in progress.

Add a CPU counter interface to allow access to a free-running counter.
It is useful to measure short time intervals. This can be used for
example to enable profiling of critical low-level functions.
Add two busy wait functions rtems_counter_delay_ticks() and
rtems_counter_delay_nanoseconds() implemented via the CPU counter.

Do not return from rtems_initialize_start_multitasking() and call
rtems_fatal() instead with a fatal source of RTEMS_FATAL_SOURCE_EXIT and
a fatal code with the exit status.
Remove all bsp_cleanup() functions. The boot_card() is now a no return
function.

This BSP now uses the MPC6xx clock driver instead of a modified PPC403 clock
driver. Support for the MMU has been added, but is not enabled by default.

Include <bsp/default-initial-extension.h> in all BSPs. Call
rtems_fatal() with RTEMS_FATAL_SOURCE_EXIT as source and the exit()
status code as fatal code in every bsp_cleanup(). Move previous
bsp_cleanup() code into bsp_fatal_extension().

bsp_cleanup() definition if necessary.

Script does what is expected and tries to do it as
smartly as possible.
+ remove occurrences of two blank comment lines
next to each other after Id string line removed.
+ remove entire comment blocks which only exited to
contain CVS Ids
+ If the processing left a blank line at the top of
a file, it was removed.

This commit covers at least PR2020, 2022, and 2023. This
patch adds all of the code for both BSPs, modifications
to libcpu/powerpc for the ppc440, and some updates to the
BSPs from follow up review and testing.
These BSPs should be good baselines for future development.
The configurations used by Ric are custom and have a non-standard
NIC. They also do not have a UART. Thus the current console
driver just prints to a RAM buffer.
The NIC and UART support are left for future work. When the UART
support is added, moving the existing "to RAM" console driver to
a shared location is likely desirable because boards with no debug
UART port are commonly deployed. This would let printk() go to RAM.
